CAC Application Process

Let's walk through the process step by step.

Step 1: Get your application packet from DORA by downloading the application from their website http://www.dora.state.co.us/mental-health/cac/CACoriginal.pdf. The application packet will clarify what is required to receive CAC certification and how to document it. Do NOT send in your application until you have met all the requirements and the application is complete.

Step 2: Completing your CAC application form

  1. The first section is a series of questions for you to answer.
    • Questions 1-6 ask for your name, address, social security number etc.
    • Questions 14 -23 asks about your past history. If you answer yes to any of these questions, you must attach an explanation describing the circumstances. Be advised that supplying false information is punishable by law.
  2. Work Verification - All hours must be signed off by a CACIII. Each level should have a record sheet. ( Level I - 1000 hours; Level II - 2000 more hours, for a total of 3000 hrs; Level III - 2000 more hours for a total of 5000 hrs).

  3. Training Education - This is where you document your successful completion of the required CAC training. Simply make a copy of your certificate of completion, which you receive at the end of each workshop, and attach copy of certificate to your application.
    • Fill out the Training/Education form by listing the workshops, the dates of the course, training category and number of clock hours. All of that information will be found on your actual certificate of completion.
    • Make sure that the order you list your certificates is in the same order you attach your copies of the certificates.
    • For those of you that are planning to substitute a college course for a CAC workshop, this is where you would document the information. First list the college course on the application then attach the course description/syllabus along with your other certificates, in the order they are listed.

  4. Verification of certification in another state. Please refer to last page of Application. If you have any other concern and or questions please contact DORA.
